You've selected "My network supports IPv4" and have not supplied a prefix, hence the complaint on the following line in red: "Please enter a valid prefix". If you don't have a prefix to enter, or don't know what it is, it's best to leave the "My network supports IPv4" box un-checked, at which point your application will go through. I'll see what I can do to make this more intuitive, but I thought that having the error message there should be enough.
