[atlas] Registering a new probe...

I have one probe (3690), and just received two new probes from Nathalie
Trenaman, thanks to Bert Wijnen. However, either I'm just flat out missing
something or the registration page is broken. I go here:

I ensure I'm logged in, and try to complete the registration process. No
matter what, it always says:


   - There were some problems with your submission. Please see below


However, nothing is highlighted or otherwise called out as an issue. I've
tried various combinations in various fields, with no success. Here's one
of the settings I've tried:
