[Accountability-tf] Draft Accountability Task Force Report - Please Review

Dear task force members,

Please find attached our first draft of the report.

It still needs further review in terms of tightening up the wording and 
making things consistent, and some sections need a little more added to 
them, but I think we've made a good start.

Please take the time to review this document ahead of our call on 
Monday, where we will need to decide whether we feel ready to move ahead 
with presenting the report at RIPE 76.

We will take comments however you want to provide them - you can send 
them to the mailing list, or send edits to the document to me directly 
and I will incorporate them.

I also have this up as a Google Doc which you can edit, but note the 
formatting isn't great here (I'll see if I can fix this up a bit):
