new documents: IP Address Request Form and Supporting Notes

Dear Local Registries,

As you probably noticed is ripe-124 ("European IP Address
Space Request Form and Supporting Notes") expired quite some time ago.
Unfortunately we had no time to revisit the document as 
extensive as we would like to do. But there are some changes to
note: 

One of the main changes is that we have split ripe-124 in two separate
documents:

- ripe-128.{txt, ps} "Supporting Notes for European IP Address Space
                     Requests"

- ripe-129.txt "European IP Address Space Request Form"

Please note that there is no ps version for the form itself anymore.
We receive too many requests by Fax, where e-mail would be possible
(and preferred). If you have any problems with this, please let us know.

Please note also other changes in the new documents. Some are listed
below: 

- nic-hdl's for admin- and tech-c's are now mandatory. Please refer to
  ripe-126 for more information.

- network details are now required for every request not only for
  requests for more than two C's

There are still a lot of changes necessary, therefor the new version
will expire again at 31st October 1995. If you have comments or
suggestions for the new document, please contact us as soon as
possible. All input is welcom.

You will receive the official notification for the two documents in
separate mails.
